Insurance cards - A copy of the front and back for each child.
Complete Immunization Records - Necessary for each child by the first Check-up.
Medical Records - Any records from a previous physician, if applicable.
Practice History
Our practice was founded over 50 years ago. It has grown from a solo practice to one that is comprised of five Physicians and two Nurse Practitioners. Pediatric Care's original location was a small brick house on Winton Road in Finneytown. The office is now located in a Professional Building just down the street from the original site. A second office in West Chester was opened in 1975. This site has relocated several times over the years and is now on Union Centre Blvd.
Prescriptions
To refill prescriptions, you can call the office during regular office hours or you can call the pharmacy. If you choose to call the office please have the patient name, date of birth, name of the medication and information, last refill date, and pharmacy phone number available. Prescriptions for ADD and ADHD controlled medications must be picked up at the office and require at least 24 hours advance notice.

Emergency
If your child is experiencing a life-threatening emergency, CALL 911 for immediate help. Other urgent problems during regular office hours can be handled by contacting the office. Notify the staff member who answers the phone that you have an emergency, and your call will be handled immediately. The physicians can be reached for emergencies after hours by calling our answering service, at (513)244-0200 and they will page the physician or nurse practitioner on call. The provider will then return your call at the number your provide. The number to our answering service is recorded on our answering machine, which you will reach when calling the office after hours.
Hospital Privileges
All of our doctors are on staff at Children's Hospital Medical Center.
